You will contribute to success by helping the patients to achieve--improved health outcomes, increased patient engagement in self-care, and improve care coordination and health care delivery.How you will make an impact: * Using Centricity, review the demographics, problems, medications, and allergies and create structured clinical care plans.* Review provider preferences and perform care coordination* Collaborate with physicians, office staff and hospital departments* Act as liaison between patient/client and Providers. Answer all patient/client inquiries for assigned Providers regarding health issues.* Answer inquiries from patients and providers and document in EMR* Execute Phone Triage responsibilities: performs a systematic, comprehensive interview of the caller/patient/client to determine the urgency of the patient's problem. * Accurately documents data for all calls and performs and documents follow-up calls with patients as necessary to help ensure positive outcomes.* Provide counseling, home treatment advice, and crisis intervention.* Maintain clinical competency through continuing education, recertification (including CPR), and clinical practice.* Work with Integrated Care Teams (ICT) to maintain open communication and maintain patient Integrated Care Plans (ICP) in MHNConnectWhat you will bring to Howard Brown Health Minimum Qualifications:* High School Graduate or GED* Active LPN license* 1-year of customer service or call center experience* 1-year clerical, data entry or medical office experience
