details) Start Date: ASAP About the Position A Program of
All-Inclusive Care for the Elderly (PACE) program in Mishawaka,
Indiana is seeking a Board Certified physician in Internal Medicine
or Family Medicine with Geriatric specialization to serve as
Medical Director . The role provides clinical leadership and
oversight for comprehensive geriatric care , focusing on frail and
elderly populations , ensuring quality clinical outcomes and
coordinated patient-centered care. The Medical Director will
supervise primary care providers and advanced practice providers,
oversee clinical operations and quality programs, and guide
interdisciplinary care teams . Responsibilities include developing
medical policies, managing chronic disease care for elderly
participants, conducting patient evaluations, and collaborating
with caregivers and healthcare teams to support holistic care
across various care settings. Shift Timings: Full-time schedule
Participation in on-call rotation Periodic regional travel as
needed Requirements Board Certified in Internal Medicine or Family
Medicine Fellowship training or Certificate of Added Qualifications
in Geriatric Medicine Indiana medical license (or willingness to
obtain) DEA and Controlled Substance certifications (prior to
start) BLS certification Minimum 1 year experience working with
frail or elderly populations Prior supervisory or leadership
experience Most recent NPDB Self Query Report (within 30 days)
Benefits Salary range: $231,000 – $280,000 based on experience
